These are actual notes taken by a human resources officer to document her investigation of allegations of sexual harassment. (All names have been changed. ) Her investigation and notes were so thorough that they were referred to by the judge in an opinion that completely absolved the employer of any liability, even when it was quite likely that some harassment occurred.
