Form 11-15. Bankruptcy Qualifying Event Notice

Generally, if an employer's bankruptcy reorganization causes a loss of health coverage (within one year before or after the bankruptcy filing) for retirees or spouses, surviving spouses and dependent children of retirees, it is a special type of qualifying event.
