Form 11-17. Notice of COBRA Coverage Rights (Loss of Dependent Child Status)

This notice can be provided to qualified beneficiaries to inform them of their COBRA rights in the event that loss of dependent child status is a qualifying event. It is based on the DOL's model notice of COBRA Continuation Coverage Rights.
