Form 11-18. COBRA Coverage Election Form (Death, Divorce, Legal Separation, Medicare Entitlement)

In the case of death, divorce, legal separation and Medicare entitlement, employers can choose to incorporate all qualifying events into this one election form. Note that the employer is required to notify the plan administrator when an employee dies.
