Form 14-26. USERRA Leave: Insurance Continuation Letter

The following letter advises employees about to commence a military leave of their rights to continuing health insurance coverage at their own expense, similar to COBRA coverage. USERRA applies to all public and private employers, regardless of size, and to all eligible employees, regardless of position.
