Form 6-20. Letter to Employee Requesting FMLA Leave

An employer may grant FMLA leave conditionally on completion by the employee's health care provider of a medical certification necessitating the leave.  The following letter explains the employee's rights and obligations, including the requirement to complete and return the certification in a timely manner.
