Form 6-23. Notice of Potential Interruption of Health Care Coverage

FMLA regulations require that group health coverage of an employee on FMLA leave be maintained by the employer for the duration of the leave. If the employee normally pays a share of the health insurance premium, he or she can be asked to continue to do so while on FMLA leave.
